Health-Related Needs Screening Tool

The Health-Related Needs Screening Tool Resources page helps health centers and other users explore and find tools that help them better understand the factors that influence health and well-being. Providers, caregivers, families, and community members can search and filter tools based on criteria such as topic area, intended audience, self-administration options, and more. The collection includes widely used questionnaires and surveys that gather information about daily circumstances, including food access, housing, transportation, and family dynamics. 

How to Use This Resource

This resource helps users explore a curated selection of screening tools that can support efforts to better understand the situations affecting a person’s health and everyday life. While the list does not include every possible tool, it features many trusted and commonly used options that support the work of health centers, schools, caregivers, and others. 

To begin, look at the Assessment Domains column to find a topic area of interest, such as housing, food access, or transportation. Use the search bar and filters at the top of the table to narrow your options by age group, language availability, self-administration, and other criteria. Whether you plan to survey students, families, or members of the community, this resource offers a strong starting point.
