The following includes a list of resources and tools to support the sustainability planning process for Hallways to Health sites. The list is organized by module and includes a description of each tool/resource and tips for using it. The tools are also discussed in the recorded module webinars.

Module 2: Engaging Stakeholders
Module 2 presents the steps to engage stakeholders to support Hallways to Health sustainability efforts. Sustainability team members (Module 1, Tool 1) should use this module to determine why it is important to engage stakeholders, which stakeholders to engage, and how best to engage each stakeholder.

Module 3: Developing a Work Plan

Module 3 presents the four steps to develop a Hallways to Health Sustainability Work Plan. Sustainability team members (Module 1, Tool 1) should use this module to develop clear goals, tasks, and timelines that will help achieve the vision for sustainability (Module 1, Tool 2).

Module 4: Introduction to Payment Reform
Module 4 introduces health care payment reform and value based payment in the context of School Based Health Centers. Sustainability Team members can use this module to familiarize themselves with value-based payment-related opportunities for School Based Health Centers and sources of support for further engagement in new payment and models of care.
Participants will be able to describe basic concepts and drivers behind value-based payment, current value based payment models, as well as how value-based payment can affect School Based Health Centers. The presentation includes recommendations on how School Based Health Centers can begin to engage in payment reform efforts and what resources are available to support them.
