The National Performance Measure data demonstrate SBHC’s role in ensuring that youth receive an annual well-child visit and risk assessment, are screened for body mass index (BMI) with nutrition and physical activity counseling, and, if age-appropriate, are screened for depression and chlamydia and receive follow-up.
